How to get UMID Card in SSS

UMID Card can now be used to transact with other government agencies such as the Government Service Insurance System (GSIS), PAG-IBIG Fund and the Philippine Health Insurance Corporation (PHIC).
Your UMID is your passport to more convenient services with various government agencies. Please take a good care of it.
Reminder: Do not laminate or alter anything on it. Do not allow others to use it. It is non-transferable. Any unauthorized use of the card may be sanctioned under the law.
1. Fill-out the Unified Multi-purpose ID (UMID) Card Application Form. You can get this form at this link: Copy and paste this link to your browser: https://www.sss.gov.ph/sss/DownloadContent?fileName=SSSForms_UMID_Application.pdf [Click Here] or get a copy at any SSS branch office information upon application.
2. Wait for you to call. The SSS agent will verify your application form together with the supporting documents or[Click Here] (See at this link: http://www.apttrendingph.com/2017/01/umid-card-application-valid-id-documents.html)
3. Once the information verified, they are going to have your digital signiture, fingerprints and take a profile picture. Once done, you will be allowed to double check the information on your profile. *No need to pay for anything, this is FREE!
4. Then they will give you the UMID application slip which bears the date when you’ll receive it. They will deliver it via mail from 3 to 6 months. But I received mine for almost 2 months only. If you did not receive the ID for 6 months, you need to go to the nearest SSS branch near on your place. Thanks
Reminder: In case of loss or change in information in the UMID Card, you can request for a replacement for a minimal fee at any SSS Branch Office.
In case of defective UMID Card. kindly surrender your card to the nearest SSS Branch Office. Replacement of defective is free of charge.
Anung gagawin kapag 6 months na wala pa ring ang UMID ID?
1. Go to the Postal Office where you have address the UMID application.
2. Kung wala doon, you need to follow-up to the nearest SSS branch office. *Note: May times na 3 months pa lang after ng application, you go to SSS branch, and when they check wala pa rin. You still just need to wait until the delivery. But after 6 months wala pa rin, try no. 1.
